Learn UX Design: The Best Interaction

As one of the coursework assignments for my Learn UX Design course, the challenge was to update a form (details below).

The wireframe is from a complex app for law enforcement and government. It’s a screen for requesting access for your organization, many of which can use the app for free, but all need special manual review. It’s based on a real draft design from a client.
Modify the wireframe to eliminate unnecessary interaction wherever possible. You are free to make other changes you think would be beneficial.
As with many of these assignments, you lack domain knowledge and access to expert clients. Think of your work as a proposal of what you’d want to ask the client about; you can presume the answers where useful.
